Interested in becoming a client of ours? Please download and fill out our two required forms. You are welcome to drop them off in person, or email to [email protected]. With those two forms, we also require we have your pets' full medical records be sent alongside them. If you do not have copies of those, please contact your previous vet(s) and have them emailed to us. Once all information has been received, our doctors will then review your pets' medical records and we will contact you for scheduling.

We require prepayment of $78 for your first appointment with us. The $78 is the appointment fee, and is fully refundable should you notify us two business days before the scheduled appointment that you need to cancel/reschedule. If you do not show for this appointment, arrive late, or cancel within two business days of the appointment, it is nonrefundable. You will be asked to prepay $78 again to reschedule. This prepayment does not cover vaccines, tests, radiographs, etc., and only the exam. We are able to take card information over the phone, or you are welcome to stop by at our front desk. We do not keep card information on file, and accept CareCredit for prepayments. Prepayment must be received at the time of scheduling.

Updated No-Show & Cancellation Policy

Many of you are aware of the struggles many businesses face with staffing shortages. Veterinary care is no different. As a result, we must limit daily appointments due to the result of our own shortages. Our mission is, and always will be, to deliver the best and most comprehensive care to your pet. That means we also have to give our staff time off to regroup & refresh themselves to do just that. We value our clients' time, and we hope our clients value not only our staff's time, but the other clients that are waiting to have their pet seen. Having a no-show appointment means we could have seen another pet that may be sick. Because we are booking further out each week, this could mean the difference for that pet and their recovery time.

As a courtesy, please call our clinic to cancel appointments you cannot make. There may be other pets waiting to be seen. We have been forced to update our No-Show & Cancellation policy due to the amount of no-shows recently. Pre-payments will be enforced, per this policy, going forward when you no-show to an appointment. If you have not yet read and signed this updated agreement, please review, sign, and bring it with you to your next appointment. We have copies at the front desk as well for your convenience.
